Nottingham City Council Whole Life Disability Occupational Therapy Team provides services to children, young people and adults within their local community. Through specialist assessment and intervention, we aim to support people of all ages to achieve their identified outcomes within their homes and short breaks environments.

We are currently looking to recruit a full time Occupational Therapist for an initial 12 month period. You will be working within a team of Occupational Therapists and Therapy Assistants and alongside Nursing and Social Work colleagues.

The role includes visiting children, young people and adults in their home and / or short break setting, and assessing for and providing equipment, adaptations and support. This includes both postural seating and moving and handling equipment.

The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under considerable pressure at times.

Service delivery is of the highest standard due to our continued commitment to quality and service improvement. To ensure that we achieve this goal, team members are supported to develop core and specialist skills through training and development; regular supervision and protected CPD time.

*Job Purpose*

Health and Social Care Officers (HSCOs) are responsible for triaging, co-ordinating and providing information and advice to citizens and professionals over the phone. This is to enable decision making and risk management on behalf of citizens requesting / requiring access to Adult Social Care. HSCOs support citizens and professionals to navigate through to the right services based on a telephone assessment of their needs.

Job Purpose*

The Oaks (St. Anns) is a modern residential unit for elderly citizens with a range of needs, including dementia. Citizens may be either short term or longer term residents, depending on their identified needs and circumstances.

Care Workers at Level 1 implement Care Plans and report any concerns or reasons care plans cannot be followed, as well as monitoring care and welfare of citizens, recording accurate information on running records, communication books, care plans monitoring charts (e.g. weight monitoring, falls, etc.) and other written and electronic communications as required.

Care Workers at Level 1 are also expected to work and cooperate with other professionals, as well as citizens and their meaningful others, to ensure a shared approach to care and support and continuity of care and treatment.

*Night workers must be able to work with a degree of independence and initiative, and be able to respond to urgent situations calmly and efficiently.*

The role is to process admission applications into school and to provide accurate advice to parents/carers, schools/academies and other services on a range of admission matters, including rights of appeal and processes involved. You will manage day-to-day casework files, maintain pupil records and deal with queries and enquires in person and over the telephone and work with colleagues to ensure a sufficient supply of school places for children and families in the area they live and where parents wish their children to go.

ESSENTIAL REQUIREMENTS:*
Ability to work under pressure and prioritise work
Ability to pay attention to detail and work accurately
Experience of computerised and record keeping systems
Experience of dealing with members of the public by telephone and in person
Ability to contribute to effective team working within a small section
