*Statement of Attorney Fees *
form updated





Recently, OAH received a suggestion to revise language on our Statement of Attorney Fees form. The *new Statement of Attorney Fees form [ [link removed] ]* has been posted on the *Forms* page on our website [ [link removed] ]. 

What changed

The updated form allows attorneys to provide more information regarding the type(s) of fees sought, including:


* Whether the amount of medical and rehabilitation benefits recovered are ascertainable for purposes of calculating contingency fees under Minn. Stat. § 176.081, subd. 1(a)(1); and
* When the amount of medical and rehabilitation benefits recovered are not ascertainable and a "Roraff-" or Heaton-type fee on an hourly basis is sought.

Additionally, the updated form more closely aligns with the form currently used by the Department of Labor and Industry.
