Recently, OAH received a suggestion to revise the language on our Affidavit of Significant Financial Hardship [ [link removed] ] form to better align with relevant Minnesota Statutes. The *new Affidavit of Significant  Financial Hardship form [ [link removed] ]* has been posted on the *Forms* page on our website [ [link removed] ]. 

What changed

The new form:


* Requires the employee to sign, rather than the employee's attorney; and
* Does not require a notary.
