Affidavit of Significant Financial Hardship

Recently, OAH received a suggestion to revise the language on our Affidavit of Significant Financial Hardship form to better align with relevant Minnesota Statutes. The new Affidavit of Significant Financial Hardship form has been posted on the Forms page on our website.

What changed

The new form:

Requires the employee to sign, rather than the employee's attorney; and
Does not require a notary.
