Out of State MD Surrenders License

OLYMPIA, WA ? In March 2023, the Washington Medical Commission (WMC) approved a Stipulation to Informal Disposition for David A. Lapides, MD (License # 61112813) in which Dr. Lapides voluntarily surrendered his Washington medical license. Dr. Lapides practices neurology and resides in Virginia.

Dr. Lapides surrendered his license after pleading guilty to felony offenses in Virginia and after receiving disciplinary action in eight other states: Alabama, Virginia, New York, New Hampshire, Florida, California, Kentucky, and Utah.
