The 2023 CIE Virtual Summit will engage subject matter experts across health, human, and social service sectors to share and learn innovative approaches to community-led care coordination.

Proposals should highlight best practices, strategies, lessons learned, or models that can be readily shared with communities planning or actively implementing a community-led care coordination effort.

All submissions must be received by no later than
11:59 pm (Pacific) on Wednesday, March 15, 2023.

Join us as we learn about Community-Led Care Coordination Models in Action! Connect with subject matter experts on various topics affecting the planning, implementation, and sustainability of CIEs. Conference sessions will highlight practical approaches for developing a CIE, including strategies for engaging local collaboratives; guidelines for building technical infrastructure that speaks to multiple systems, and tips for developing a shared governance infrastructure.
