West County Health Centers is a nonprofit federally qualified health center in Sonoma County, California.
Their newly launched behavioral health program, called Collaborative Approach to Recovery and Empowerment and known as CARE+,
provides psychosocial rehabilitation to West County’s high-risk patients who face mental health challenges.
CARE+ is managed by Maximon Behavioral Health, a Southern California–based for-profit behavioral health services provider.
However, in this case, it operates within West County Health Centers to send community health workers to help patients with basic life skills. CARE+ is an example of whole-person care that provides wraparound services to patients.
