An exciting new opportunity is now available within Nottingham City Council Crocus Fields Outreach service for 18.5 hours (two years). We are seeking a dynamic, innovative, enthusiastic and motivated experienced Outreach Worker who is passionate and committed to providing support to young people and their families. The Crocus Fields Outreach Service is a uniquely integrated service delivering a range of interventions to disabled young people and their families. The outreach worker will provide assessment and the delivery of behaviour management programs to disabled children, young people and their families. This service ensures that disabled children are enabled to be continued to be cared for at home by their parents/carers by addressing challenging behaviours that result from their disability. ** *
Responsibilities include:* ** The role involves a significant amount of direct intervention work with families on issues such as parenting, behaviour, sleep, health and integration. The role requires an ability to establish positive relationships with young people and families, assessing needs and formulating and delivering behaviour management programmes. The Outreach worker will be expected to contribute to effective multi-agency working with other professionals, to provide individual and group work for disabled young people and their families/carers.

You should have knowledge of child development, children's act 1989, child protection policies/procedures and issues that affect disabled children and their families. Understanding the importance of maintaining professional boundaries and equal opportunities, anti-oppressive practice and the impact of discrimination is also essential. Experience of working with disabled children and families is essential.

You will need to have experience of managing cases or offering direct interventions; working in partnership with a wide range of professionals and developing behaviour plans. You will need to have good communication skills, both written and verbal, including report-writing and presentation skills, prioritising and organising workload, adhering to service timescales and providing a timely, responsive service to families. Flexible working is also essential in this role as is working on your own initiative, adherence to safe lone working practices and experience of using Microsoft Office & outlook packages are also essential.
