To assure care coordination for mobile patients, MCNs Health Network offers continuity of care and treatment completion by providing comprehensive case management, medical records transfer, and follow-up services for mobile patients. During this webinar, you will learn how to enroll a patient in Health Network, strategies for incorporating Health Network into your existing clinic systems, and the steps Health Network associates take to ensure continuity of care for your patients while keeping your clinic informed of patient outcomes. Health Network has proven to be an easily modifiable patient navigation system for healthcare needs of mobile populations with application in disease surveillance and treatment management for any number of injuries, illnesses, or care needs among mobile populations.
Presenters
Laszlo Madaras, MD, MPH

Laszlo Madaras, MD, MPH, is a hospitalist in Pennsylvania and the Chief Medical Officer for Migrant Clinicians Network. As CMO, he oversees all MCN's clinical activities, including Health Network. Dr. Madaras has extensive experience with immigrant, migrant, and refugee populations. As a child refugee, he emigrated with his family to America. He has worked in refugee camps around the world and has worked closely with immigrant and migrant families in his community to support their health and well-being. He received his MD and Master's in Public Health from Tufts University School of Medicine. He is also a staff physician in Tuberculosis control for the Pennsylvania State Health Department, Clinical Assistant Professor of Medicine at Penn State College of Medicine, and Adjunct Assistant Professor of Family Medicine at Brown University. 
Camila Velasquez 

Camila Velasquez is a Health Network Associate for Migrant Clinicians Network. As a Health Network Associate, she provides continuity of care services for patients who are highly mobile and at risk for loss to follow up at the health center. Camila Velasquez (she/her/ella) moved from Cartagena, Colombia to Miami, Florida with her family at the age of two. At the age of ten, Velasquez and her family had to move back to Colombia due to her father’s expired work visa. At the time she did not understand why she was so different from her peers. Her experiences going through the immigration process at a young age and acquiring citizenship at the age of nineteen put her on the path to becoming an advocate for her community. She graduated from Florida International University in Miami, Florida with a Bachelor’s in Psychology with aspirations of providing services to those in need. Velasquez is a strong mental health advocate which drives her to help the diverse community that surrounds her. 
Joshi Covarrubias 

Joshi Covarrubias is a Health Network Associate for Migrant Clinicians Network. As a Health Network Associate, he provides continuity of care services for patients who are highly mobile and at risk for loss to follow up at the health center. Joshi Marth Covarrubias moved from Santa Rita in Jalisco, Mexico to Grand Rapids, Michigan, with his mother and younger sibling as a child. His experiences as a community advocate for the local Spanish-speaking community, including assistance in finding health services and gaining legal status, spurred his interest in working for MCN. He graduated from Cornerstone University in Grand Rapids, Michigan with a Bachelor’s degree in Kinesiology. He is the first person in his family to attend and graduate from college. He is also an Army veteran. Covarrubias recently moved to Austin with a goal of attending graduate school at Texas State University.
