In partnership with the Asthma and Allergy Foundation of America’s Health Equity Advancement and Leadership (HEAL) program, Breathe Southern California has launched its Breathe Easier ™ Asthma Management (BEAM) program. BEAM is an asthma home remediation program for adults in a culturally appropriate case management format designed to address the needs of adults with asthma in Los Angeles County.
Participants will receive personalized, in-home asthma guidance from a health educator to help with the following:
  • Managing their asthma
  • Addressing in-home environmental concerns and potential asthma triggers
  • Connecting patients with resources
Who is Eligible?
  • Adults diagnosed with high-risk asthma who live in Los Angeles county and are enrolled in Medi-Cal
Requirements:
  • Patient consent to join the program
  • Obtain a provider's referral, or have the provider's office sign Breathe SoCal's asthma confirmation letter
  • Be willing to conduct three (remote or in-person) visits with a health educator over a one-year period.
  • Conduct four 20-minute surveys (at the program's start, halfway, at the end of the program, and one year later.)
In addition to personalized guidance from a health educator, participants will receive the following:
  • Educational materials
  • Bedding cover
  • Sweat X™ Free and Clear Detergent
  • Rabbit Air™ HEPA air purifier (while supplies last)
  • A $50 gift card for completing each survey
