*Nottingham City Council is recruiting a Contract and Commissioning Officer to join the Children in Care Placements Contract Management and Commissioning function in support of a new Fostering and Residential Care Framework across the D2N2 partnership.

* Supporting the four partnership Local Authorities Derby City, Derbyshire County, Nottingham City and Nottinghamshire County Councils you will coordinate contract management of the framework across the region, identifying and developing opportunities for efficiencies and service improvements. This will involve both the analysis of trends in usage and working with providers to promote on-going market development. Key to the role will be supporting operational colleagues from across the partnership to seek improved outcomes for children and young people and helping more young people to live closer to home.
You will need an understanding of the private market for fostering and childrens residential care, and of Local Authority commissioning and contract management. *Job Purpose

To provide an excellent customer focused service to citizens, carers, other colleagues and partners requesting Occupational Therapy or other services.

To ensure citizens' and carers' needs are assessed and triaged at the point of contact with the supporting guidance of a senior colleague and that citizens and carers are advised of the services available to them.

To liaise with Social Care and Health professionals as appropriate to ensure that citizens and carers are triaged to determine urgency and presenting level of risk.

To signpost citizens, carers, other colleagues and partners to appropriate provision and services, dependent upon need.

*The daily activities of the role are varied, but may include:

* Taking referrals via telephone, email or letter;

General administrative duties including scanning, typing and filing;

Liaising with professionals, both from other teams within Occupational Therapy and Adult Social Care and external partners;

Updating records when required;

Signposting citizens and carers to relevant services;

Any other ad-hoc tasks as directed by senior colleagues.

*Job Purpose

* The day services at the Indian Centre provide facilities and support to older people, with the focus on developing, maintaining and preventing the loss of, a person's independence, by enabling people to rebuild their confidence, increase socialisation and an overall improvement in their quality of life. The service is hosted within the ICCA and is specifically tailored to meet the religious, cultural and social needs of citizens from an Indian background.
Care Workers at Level 1 implement Care Plans and report any concerns or reasons care plans cannot be followed, as well as monitoring care and welfare of citizens, recording accurate information on running records, communication books, care plans monitoring charts (e.g. weight monitoring, falls, etc.) and other written and electronic communications as required.
Care Workers at Level 1 are also expected to work and cooperate with other professionals, as well as citizens and their meaningful others, to ensure a shared approach to care and support and continuity of care and treatment.
*Please be aware that this is an exempted post for a **female of Indian Origin, under the "Equality Act 2010 - Section 83, Schedule 9 Works Exemptions, Part 1 Occupational Requirements, Paragraph 1. *SHORT BREAKS*

An exciting opportunity has arisen to join our Short Breaks Link Work Team. Our team support children and young people with disabilities and complex health needs to access their community, develop friendships and gain independence. As part of the Short Breaks Link Work Team you will have the opportunity to access a wealth of training in order to develop your skills and work as part of a supportive team to deliver the best services possible to the young people we support. The role offers the opportunity to become an important part of young people's lives and enable them to achieve their aspirations. As their allocated link worker, you will support young people on a weekly basis aiming to achieve identified outcomes and developing independence, by taking part in a variety of fun and exciting activities.

Working with young people with Autism, Learning Disabilities, Physical Disabilities and complex health needs, successful candidates will be supported to develop their knowledge to become all around experienced support workers. The role supports Children and Young People who live within the Nottingham City area and the post holder will offer direct support and care both in the young persons home (where appropriate) but mainly in their community.This support will be on a 1:1 basis; however, some young people will require 2:1 support, dependent on assessed need.

Successful candidates will possess knowledge and experience of supporting children and young people who can demonstrate challenging behaviour. They must have a proactive 'can do' approach and the ability to think creatively when planning Link Work Sessions. Candidates should be physically able to carry out all the duties of the role; i.e. supporting complex needs and challenging behaviours.

Candidates should ideally have their own mode of transport, or be able to use public transport for the efficient performance of duties in line with service requirements.
