Patients leaving a health center’s service area in the midst of treatment are often lost to follow-up. Many are unable to keep up their medications, which threatens their health. That’s why hundreds of clinicians in health centers around the country turn to Health Network, a free service that assists health centers by providing individual case management to individuals with any ongoing health concerns who need to leave the service area.

Since 1995, Health Network has assured continuity of care and treatment completion by providing comprehensive case management, medical records transfer, and follow-up services for mobile patients. As rapid travel and ease of mobility make it possible to arrive anywhere in the world in a matter of hours, and as shifting economic conditions require individuals and families to move in order to stay employed, Health Network has proven successful in providing individualized case management and patient navigation for any injury, illness, or care need to mobile patients.

This webinar will provide you with the essential information you need to begin using Health Network for your mobile patients. You will learn how to enroll a patient in Health Network, strategies for incorporating Health Network into your existing clinic systems, and the steps Health Network associates take to ensure continuity of care for your patients while keeping your clinic informed of patient outcomes.
