Breathe Southern California is looking for individuals to participate in its 2022 Community Asthma Needs Assessment. This survey should take approximately 10 - 15 minutes to complete and can be accessed using this link.
This survey will assess the needs of those living in the Southern California region with asthma, help prioritize their unmet health needs, and act as the foundation for future asthma initiatives.
Individuals who submit a survey will be entered to win a $25 Target gift card.
