Our Practitioner Performance Advice service has today published its latest Insights paper: Behavioural Assessments: findings from an in-depth qualitative and quantitative analysis and an accompanying video outlining the behavioural assessment process. Learning from medication errors – anti-infectives

We have published the final instalment in a suite of five ‘Did You Know’ information leaflets dedicated to sharing learning from medication errors. This fifth leaflet shares learnings from 172 claims linked to anti-infectives medication. Such medication includes antibiotics, antivirals and antifungals. The entire medication errors series can be found on our website.

NHS Resolution collaborates on surgery best practice guides

NHS Resolution has worked with Getting It Right First Time (GIRFT), the Royal College of Surgeons of England (RCS), the Association of Surgeons of Great Britain (ASGBI), and the British Association of Endocrine and Thyroid Surgeons (BAETS) to create the guides, outlining how clear documentation of procedures can support the resolution process and provide the best care for patients. Case of note: MT and KT v. Kent & Medway NHS Social Care Partnership Trust (High Court, 10/2/2022 – Johnson J.)

This unusual case involved both a claim for clinical negligence and a separate claim under the Human Rights Act. After a father suffered a psychotic episode and stabbed his daughter causing serious injuries, they took the trust that had discharged the father from secondary care to court. Ultimately, the court found in favour of the defendant with regard to both claims. Read more…
