Changing party names





When a new case is filed at OAH, C-Track creates a case caption using the party names listed on the initiating pleading. Sometimes, that information is inaccurate. For example, maybe the filer entered ?SFM? as the insurer, but the correct entity is ?SFM Mutual.?

*If a party name is incorrect, there are three ways to change it:*

* Notify the employee?s representative so that an *amended petition* can be filed; *or*
* File a written *Motion to Amend Caption* for the judge?s review; *or*
* Make a motion to amend the caption *on the record* at the hearing.

*Can I just call or email OAH?*

No. Informal communication is not made a part of the case record. Case parties are responsible for ensuring case captions are accurate. When your request is made through a formal motion, other parties can respond.
