Medicare Advantage And Dialysis

Enrollment by patients with end-stage renal disease in Medicare
Advantage (MA) plans is likely to grow rapidly because of a recent
federal policy change.

The 21st Century Cures Act of 2016 lifted regulations prohibiting MA
enrollment after patients initiate dialysis, starting January 2021.
Experts anticipate MA uptake to increase by more than 60 percent by
2026.

Eugene Lin and coauthors find that the largest MA plans pay about
one-quarter more than Medicare fee-for-service rates for dialysis

during 2016-17, with the highest markups paid to large dialysis chains.

The authors' findings contrast those from nondialysis settings, where MA
provider payments are similar to or lower than those for fee-for-service
Medicare.

Overall, the authors suggest that broader reforms aimed at increasing
market competition might be required to address payment disparities.
