There are full time and part time hours available in all teams.

The starting salary is ?25,295 (Level One) rising to ?26,999 (Level Two) on the first year anniversary, subject to satisfactory completion of a probation period (pro rata for part time workers).

* Candidates could earn up to ?32,878 depending on post qualifying accreditation.

Team Information

?

The Community Prevention Team works with citizens who have multiple long term conditions (LTCs), which include citizens with neurological disabilities, and those who require ongoing ?case management? in relation to their complex needs or support, in order to reduce the risk of avoidable admissions to hospital or long term care.?

?

Colleagues in Community Prevention are linked with a Care Delivery Group and work collaboratively with the multi-disciplinary teams (MDT) responsible for allocated citizens in that locality.? Colleagues are also involved in the development and delivery of Community Led Support (CLS) in Nottingham in their CDG area and will help to develop the MDTs? understanding of and support for this.

?

The Placement Review Team is responsible for reviewing the ongoing care needs and placements of citizens who are placed in residential and nursing homes. This may be a regular annual review, or it may be an unplanned review if new concerns are raised or other circumstances change. Placement Review Team members may also be involved in the Home Closure process, in partnership with Adult Safeguarding and Quality Assurance.

Social Workers in both Community Prevention and Placement Review may be expected to complete Court of Protection (CoP) work, depending on their experience.

?

The Integrated Enablement Team is split into two functions: The Integrated Discharge part of the team is hosted within Nottingham University Hospital and is a multiagency team facilitating discharge for citizens following an acute hospital stay. Colleagues work as part of a multiagency system to identify the discharge Pathway and facilitate short-term services to maximise independence/rehabilitation and reablement.

The Out of Hospital part of the team is a community-based team, which supports citizens with assessment, review and planning for longer-term support services following discharge from an acute hospital stay. Colleagues are part of a multiagency service working with other allied professionals and clinicians to identify long-term support needs for citizens and commissioning services to provide this support.

All Social Workers

?

Nottingham City Council Adult Social Care offers a robust ASYE programme for newly qualified Social Workers, and other various development programmes for all Social Workers, such as AMHP qualifications and Practice Educator status. All permanent Social Workers are given the opportunity to rotate into different areas of Adult Social Care, and take part in project work at a strategic level when available.

  1. CQSW / CSS / Dip SW, Degree Qualification and HCPC Registration;

  4. Experience of carrying a full caseload and undertaking assessments of need with minimal supervision (L2 and L3)

Nottingham City Council are looking to appoint a Cook at Crocus Fields short breaks unit for children and young people with disabilities. Crocus Fields offers care to young people who have severe learning disabilities and physical disabilities, many of whom are on the autistic spectrum.

The role requires an ability to provide a flexible catering service to meet the needs of service users and staff. The principal duties will include providing meals to meet varied dietary needs of the young people, order and purchase food, menu planning, managing the food budget, offering nutritious and well balanced meal, and complete all the necessary legislative administrative tasks and domestic duties. This post is predominately one where you will be expected to work using your own initiative.
