Dear John,
North Carolina is one of several states in the USA that requires a “certificate of need” (CON) before a physician group could build an outpatient procedure center?
Along with other states, like Massachusetts and Illinois, the rules of North Carolina require that a CON be obtained before a licensed surgeon or physician, or a medical practice group, can build and operate an outpatient surgery or procedure center. This would be done using the physician's own funds or personal loan, not taxpayers’ money. The process for obtaining this CON is onerous, involving much time and financial resources, and is almost never successful, as large hospitals or corporations which own hospitals block these applications.
When outpatient surgical centers are built and owned by physicians or physician groups, they can provide care, such as cataract surgery and even total hip and knee replacements, much more efficiently and at less expense to the patient (customer) than a large hospital! Any profit realized from this efficiency goes to the physicians, not to hospital or corporation bureaucrat coffers.
Enterprising physicians in Chicago, after being denied a CON by the state of Illinois, built outpatient surgery centers across the state lines in both Indiana and Wisconsin to provide convenient, cost-effective, and lower hassle care for their patients. The same has occurred in Boston, where physicians built a surgery center in New Hampshire. These are just two examples of how the
“free-market” approach to medical care can both improve quality patient care and lower health care costs.
As a nurse who wants to advocate for quality affordable healthcare, I will work to decrease regulation without compromising quality measures to promote innovation and success in seeing improvements in our healthcare system.
