Team Information:
The Whole Life Disability Programme supports citizens with physical and learning disabilities from childhood right through to adulthood to ensure a smooth transition between services. The team works closely with Health colleagues to commission and support citizens in a range of services including community support, residential and hospital settings.

Nottingham Supported Living Service is providing sustainable community housing with support opportunities for adults with Mental Health support needs. The team is supporting citizens to either move to supported living from their current residential accommodation, or to prevent them from having to be admitted into residential care. Working in this team will involve strength-based assessments of potential tenants, completing Capacity Assessments and (where required) leading on Best Interest decision making processes in compliance with The Mental Capacity Act 2005.

Mental Health Social Care Teams are specialist teams that commission and review a wide variety of packages of care for citizens who are in contact with secondary mental health services, with the aim of promoting independence and wellbeing.
????
In addition to undertaking initial assessments and submitting packages of care for approval, Social Workers regularly write reports for Mental Health Review Tribunals, undertake Mental Health Act Assessments (Approved Mental Health Professionals) and formally assess the capacity of citizens to make key decisions about different aspects of their life.

Occupational Therapy is a specialist branch of Social Care that assesses and reviews adults, children and young people who may have impairments to their mobility or motor skills. These may be long or short term, and require an assessment and care plan to be put in place, often with equipment prescribed.

Occupational Therapists are expected to ensure the delivery of appropriate services to adults, children, young people and their families / carers, according to their assessed needs. This will include assessing the needs of disabled children and their carers against the Department?s agreed eligibility criteria and in accordance with National Performance Indicators within set timescales, as well as the creation and implementation of culturally appropriate care plans that demonstrate flexibility and innovative use of resources.

Occupational Therapists are also responsible for delivering good outcomes for disabled adults, children and young people through completing timely assessments, and developing and implementing effective care plans, including provision of equipment, postural seating and structural adaptations that safeguard vulnerable people. They should work within the legislative framework at all times, and in partnership with internal and external partners and agencies.

Occupational Therapists are expected to maintain and produce professional case records and reports which are structured, clear, evidence based and logical in accordance with the Department?s case recording standards and practice. In addition, they should make best use of the Department?s information systems, both using information from the relevant Registers and inputting new information as required in a timely and accurate manner.

The JackDawe team provide a specialist service supporting citizens with a range of complex needs, to enable them to continue with everyday activities and tasks that are meaningful to them. This also encourages independence and can reduce the impact of behavioural and psychological symptoms associated many complex needs. JackDawe service also supports in practical ways, such as medication management, personal care and home management.

Both teams encourage independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen?s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.
