There is growing momentum in strengthening how the health care, public health, and social services systems all work together to support people in living their healthiest possible lives. But there is also increasing confusion over what health care's role should be in addressing the social determinants of patients' and community-wide health.

RWJF is interested in developing a set of clear principles to set the standard for the health care sector to provide equitable care that fully addresses people's physical, emotional, and social needs. Along with this, RWJF hopes to create guidance for implementing care delivery that is fully responsive to patients' goals, needs, and life circumstances.

Organizations may apply individually or as a collaboration of two or more organizations. Up to $5 million is available.
