We invite you to join one of the following four learning collaboratives within the Special and Vulnerable Populations Diabetes Task Force. The learning collaboratives build upon the National Learning Series, which was hosted during Fall 2021.
SELECTION CRITERIA
Health centers and health center look-alikes will be prioritized for selection. Given our limited capacity per learning collaborative, the Task Force requests that health center organizations indicate ALL of the learning collaborative sections which work for you (based on topic and your availability). We will then notify you about which learning collaborative you have been selected to participate in.
