Farmworker Health Network is seeking data related to health centers that are implementing services or partnering with organizations to address SDOH.
The Farmworker Health Network members are working on grant activities for the HRSA National Health Center Training and Technical Assistance Partners (NTTAPs) award in providing national training and technical assistance (T/TA) for migrant health centers. HRSA has prioritized Social Determinants of Health (SDOH), asking NTTAPs awardees to support health centers in addressing these social determinants for their patients. SDOH are the factors that affect a person's health and well-being beyond just the quality and accessibility of health care - commonly outlined broadly as issues surrounding a person's access to quality housing, education, employment, transportation, food security, and other topics.
In order to fulfill HRSA's request and ensure FHN members are providing quality T/TA to all migrant health centers, we are seeking data related to health centers with migrant funding that are currently implementing services or partnering with other organizations to address SDOH.
