The starting salary is ?19,171 rising to ?19,945 on first year anniversary, subject to satisfactory completion of a probation period. This is pro rata for part time hours.

Hours will be worked according to a rota, and this will include weekend work. A valid driving licence and access to your own vehicle is essential for this role.

Nottingham City Council Homecare provides two services; Social Care Reablement and JackDawe.
??
Social Care Reablement is a specialist Homecare team that works across the city to provide support to citizens in their own homes. This is mostly acute, short term care for citizens who have recently been discharged from Hospital or another service. The focus of the work done by Social Care Reablement is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.

The JackDawe team provide a specialist service supporting citizens with a range of complex needs, to enable them to continue with everyday activities and tasks that are meaningful to them. This also encourages independence and can reduce the impact of behavioural and psychological symptoms associated many complex needs. JackDawe service also supports in practical ways, such as medication management, personal care and home management.

Both teams encourage independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen?s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.
???
ESSENTIAL REQUIREMENTS:
? Ability to assess social care and health needs and develop, implement, maintain and update Person Centred Care Plans;
? Experience of, or ability to, provide support and guidance to colleagues and to be able to deal with emergency and difficult situations;
? Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds;
? Ability to administer medication in line with departmental policies and procedures.

Job Purpose:
Occupational Therapists at all levels are expected to ensure the delivery of appropriate services to adults and their families / carers, according to their assessed needs. This will include assessing the needs of vulnerable adults and their carers against the Department?s agreed eligibility criteria and in accordance with National Performance Indicators within set timescales, as well as the creation and implementation of culturally appropriate care plans that demonstrate flexibility and innovative use of resources.

Occupational Therapists at Level 1 are also responsible for delivering good outcomes for vulnerable adults through completing timely assessments, and developing and implementing effective care plans, including provision of equipment and structural adaptations that safeguard vulnerable adults. They work within the legislative framework at all times and in partnership with internal and external partners and agencies.
????
Occupational Therapists at Level 2 are also expected to develop more complex person centred packages of support; this may include rehabilitation / reablement, provision of equipment and structural adaptations. They will carry a full caseload, including multifaceted cases and complex manual handling assessments, and will work with minimal supervision.
????
Occupational Therapists at Level 3 are also expected to provide reports to safeguarding strategy meetings or review meetings as required, as well as managing multifaceted cases (including those requiring complicated equipment and major structural adaptations that keep people and their carers safe).? At this level, there is a need to have substantial moving and handling experience and the need to support less experienced colleagues.?
