*There are various permanent positions available for Short Breaks Link Workers, to work either 9.25 or 18.5 hours predominantly at evenings and weekends.*

*The starting salary is 21,589 rising to 22,911 on first year anniversary, subject to satisfactory completion of a probation period (pro rata for part time).*

*SHORT BREAKS*

An exciting opportunity has arisen to join our Short Breaks Link Work Team. Our team support children and young people with disabilities and complex health needs to access their community, develop friendships and gain independence. As part of the Short Breaks Link Work Team you will have the opportunity to access a wealth of training in order to develop your skills and work as part of a supportive team to deliver the best services possible to the young people we support. The role offers the opportunity to become an important part of young people's lives and enable them to achieve their aspirations. As their allocated link worker, you will support young people on a weekly basis aiming to achieve identified outcomes and developing independence, by taking part in a variety of fun and exciting activities.

Working with young people with Autism, Learning Disabilities, Physical Disabilities and complex health needs, successful candidates will be supported to develop their knowledge to become all around experienced support workers. The role supports Children and Young People who live within the Nottingham City area and the post holder will offer direct support and care both in the young persons home (where appropriate) but mainly in their community.This support will be on a 1:1 basis; however, some young people will require 2:1 support, dependent on assessed need.

Successful candidates will possess knowledge and experience of supporting children and young people who can demonstrate challenging behaviour. They must have a proactive 'can do' approach and the ability to think creatively when planning Link Work Sessions. Candidates should be physically able to carry out all the duties of the role; i.e. supporting complex needs and challenging behaviours.

Candidates should ideally have their own mode of transport, or be able to use public transport for the efficient performance of duties in line with service requirements.

*Essential Requirements*


* Experience of working with disabled children, young people and their families;
* Experience of identifying and supporting others to deliver positive outcomes for children and young people;
* Ability to demonstrate a can do approach that will give confidence to linkers/carers and families we are working with;
* To respond promptly to incidents that may have child protection implications and refer appropriately to case holder.
