To ensure care coordination for mobile patients MCN’s Health Network works to make certain that migration is never an impediment to health when continuity of care for treatment completion is needed. By providing comprehensive case management and medical records transfer, Health Network provides essential follow-up services for mobile patients. During this webinar, you will learn of the simple requirements for patient enrollment, review strategies for incorporating Health Network into your existing clinic systems, and the processes employed by Health Network associates to ensure continuity of care for your patients while keeping your clinic informed of patient outcomes.
