Considering Health Spending
The quality-adjusted life-year (QALY) is a standard measure that
incorporates life expectancy and quality of life and is often used in
cost-effectiveness analysis.
Leah Rand and Aaron Kesselheim conducted a systematic literature review
of the types of criticism leveled at the QALY and concluded, "The nine
major criticisms of QALYs fall into three categories: methodological
concerns, criticisms of QALY neutrality, and potential
discrimination."Â Â
Another health spending study examined high hospital price regulation.
While consolidation has been shown to yield higher prices, Maximilian
Pany and colleagues found that, after adjusting for area wages,
high-price hospitals
are prevalent across the concentration spectrum and most are in
unconcentrated or moderately concentrated markets.
"Limiting policy actions to concentrated hospital markets, particularly
when those markets are defined broadly, would likely result in poor
targeting of high-price hospitals," they concluded.
