Adult Duty
The Adult Duty Team is a busy service comprising Social Workers and Community Care Officers. The team works with a diverse range of citizens and carers to provide holistic strength-based assessments and reviews to maximise independence and reach outcomes, identifying appropriate services to meet needs and commissioning support where required.
The team works with citizens in a variety of different situations, including where a crisis response is required, and works with multiple agencies to provide risk assessed responses that take account of individual needs and outcomes, with consideration of mental capacity matters throughout.

Community Prevention
The Community Prevention Team works with citizens who have multiple long term conditions (LTCs), which include citizens with neurological disabilities, and those who require ongoing ?case management? in relation to their complex needs or support, in order to reduce the risk of avoidable admissions to hospital or long term care.?
Colleagues in this team are linked with a Care Delivery Group and work collaboratively with the multi-disciplinary teams (MDT) responsible for allocated citizens in that locality.? Colleagues are also involved in the development and delivery of Community Led Support (CLS) in Nottingham in their CDG area and will help to develop the MDTs? understanding of and support for this.

Nottingham City Council Adult Social Care offers a robust ASYE programme for newly qualified Social Workers, and other various development programmes for all Social Workers, such as AMHP qualifications and Practice Educator status.

??* Community Care Officers are required to complete Care Act assessments; face to face or telephone assessments, depending on the level of risk and the needs of the citizen being assessed, with consideration of mental capacity throughout.? They work with supportive colleagues, citizens, carers, statutory and community partners to provide appropriate support that will enable independence, choice and control, promoting early intervention and local community led support services to enhance well-being and respond to local community needs. ?? The CCO role is interesting and varied, and deals with risk management on a daily basis.
