As a Specialist Navigator, you will offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following discharge from hospital. You will deliver a tenacious approach to build trust and rapport and maximise positive outcomes for some of the most persistent, hard-to-engage rough sleepers in Nottingham and Nottinghamshire enabling them to move away from (and avoid a return to) street homelessness.

ESSENTIAL REQUIREMENTS:?
? Experience of supporting individuals with complex needs (e.g. rough sleeping, substance misuse, physical and mental health needs)
? Ability to identify people?s strengths and understand the impact of events in life on the individuals presenting behaviour (e.g. trauma)
? Understanding of the wider issues impacting upon homelessness, the barriers facing people with multiple and complex needs and the challenges involved in resolving rough sleeping.
? Knowledge of local social and healthcare services and ability to navigate systems and pathways into services
? Excellent interpersonal skills and the ability to maximise the value of those relationships to deliver jointly agreed outcomes
? Skilled in working in a creative and innovative manner to overcome barriers and ensure outcome

Casual Residential Night Care Assistant at Wood View Children?s Home for children and young people with disabilities. Wood View offers permanent residential care to young people who have severe learning disabilities and physical disabilities, many of whom are on the autistic spectrum and who may exhibit significant challenges in management of their behaviour. The role requires an ability to:

To give essential and personal care to children and young people with autism and learning disabilities. To establish positive relationships with the young people, assessing needs and risk.
?
To support young people during the night and support them in their bedtime routines.
?
To support young people with their medical and all round health needs.
?
To ensure a high standard of supervision and support is provided to assist children and young people with Autism and learning disabilities.
?
Applicants must have experience of working with people with disabilities.
?
The potholders primary responsibility, will be to ensure the physical safety and security of the children and young people during the night hours
?
You will need to respond to the needs of the young people with a wide range of learning disabilities and will be required to undertake domestic tasks and have general oversight of the building security.

Essential Requirements
  • Ability to work in partnership and maintain professional relationships with young people, their families and relevant others.

  • Ability to work a rota covering shifts from 10pm to 7am.
