*Job Purpose:* The Senior Practice Officer will co-ordinate a multi-agency discharge pathway for children who are part of the Transforming Care Programme, and will work alongside the Whole Life Disability Team Managers, Nottinghamshire CCG and Nottinghamshire Healthcare Trust to ensure timely discharge from hospital is achieved. They will also act as point of contact for the Local Authority, for placements at risk of breakdown or where an alternative care pathway needs to be identified. The main duties of the post will include working directly with the families of children with Autism/ASD, and significant additional mental health needs. It will involve working with children currently in hospital settings, as well as those at risk of hospital admission, with a wider remit of children with Autism/ASD who are impacted significantly by their mental health.
The successful candidate will be responsible for providing direct support to children and families, and advice, guidance and training support to agencies involved with them, to provide a clear and co-ordinated package of support. There will be an emphasis on inter-agency working, with a duty to highlight and recommend ways in which services can be offered. *

* *Essential Requirements* Appropriate professional qualification e.g. CQSW, DipSW, CSS, OT or Nursing degree, as well as relevant professional registration;
Thorough understanding of the best social work practice and recent research in respect to disabled children and children with mental health difficulties;
Understanding and ability to apply national and local priorities, drivers and legislation to facilitate continued government visions for change in social care;
Ability to work effectively with children and families as well as a range of agencies and professional and community groups, in order to deliver good outcomes for children and parent/carers.
