An exciting chance to join a large Occupational Therapy team within adult social care as the department begins a three year service transformation process, providing the opportunity to be involved in shaping the services of the future

The Adult Occupational Therapy Team covers a variety of services including Early Intervention, Re-ablement, Rapid Response and Disability Housing Advisors as well as its mainstream function. The team focus is on promoting independence and well-being, through prevention, re-ablement and early resolution, having a strength based approach and putting the person at the centre of all interventions.?

There is an expectation that Occupational Therapists at all levels within the service will ensure the delivery of appropriate services to adults and their families /carers, according to their assessed needs whilst Working within the current legislative framework at all times, and in partnership with internal and external partners and agencies. ?This will include assessing the needs of vulnerable adults and their carers, establishing what is important to them and agreeing outcomes based on a personalised, sensitive and proportionate approach. Creating and implementing culturally appropriate care plans that demonstrate flexibility and innovative use of resources.

There is also an expectation that Occupational Therapists at all levels will maintain and produce professional case records and reports which are structured, clear, evidence based and logical in accordance with the Department?s case recording standards and practice. In addition, they should make best use of the Department?s electronic information systems, inputting new information as required in a timely and accurate manner.

The Senior Practitioner will be responsible for assisting the Team Manager in developing and promoting a team culture of continuous improvement and performance management, advocating a problem solving, positive risk taking teamwork approach that enables the best possible outcomes for citizens and their carers. By providing continuous review of new and current referrals, the senior practitioner will ensure the most effective service provision based on needs whilst ensuring an effective contribution to departmental targets, local and national performance indicators.

The Senior Practitioner will also provide supervision, leadership and appropriate professional governance to a group of up to eight Occupational Therapists, Community Care Officers and team administrators as agreed with Team Manager.

Essential Requirements
  1. Diploma / Degree in Occupational Therapy and registration with the Health & Care Professions Council (HCPC);
  2. Substantial experience as an Occupational Therapy practitioner with disabled people and families / carers across a range of functions, including assessment, provision of equipment and major adaptations;
  3. Thorough knowledge of the relevant legislation pertaining to Occupational Therapy in respect of health and social care including; equipment, adaptations, housing, and safeguarding;
  4. Evidence of leadership skills and the ability to promote team effectiveness in a changing environment.

Job Purpose
Social Care Reablement is a specialist Homecare team that works across the city to provide support to citizens in their own homes. This is mostly acute, short term care for citizens who have recently been discharged from Hospital or another service. The focus of the work done by Social Care Reablement is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.

The team encourages independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen?s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.
? ?
Care Workers at L3 support colleagues at L2 to provide safe and appropriate personalised care and support to citizens. This includes writing and maintaining individual person centred care plans, completing assessments and the administration of medication.
? ?
The successful candidate will be required to engage in citizens? meetings when needed, to ensure that the care package is always up to date and communicated to all relevant stakeholders as well as undertaking direct observations of care delivered, making recommendations of how to improve care provision and using feedback to adjust the care plan as required.

Essential Requirements
  1. Ability to assess social care and health needs and develop, implement, maintain and update person centred Care Plans;
  2. Ability to administer medication (create medication administration records, book in medication and complete returns and follow departmental policies and procedures) and conduct medication competency assessments;
  3. Excellent oral and written communication skills (including facilitating reviews, engagement, report writing, maintaining good quality records, letters, difficult conversations etc.)
  4. Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds.
