The starting salary is 22,183 (Level One) rising to 23,541 (Level Two) on the first year anniversary, subject to satisfactory completion of a probation period (pro rata for part time workers).*

Job Purpose* Health and Social Care Officers (HSCOs) are responsible for triaging, co-ordinating and providing information and advice to citizens and professionals over the phone. This is to enable decision making and risk management on behalf of citizens requesting / requiring access to Adult Social Care. HSCOs support citizens and professionals to navigate through to the right services based on a telephone assessment of their needs. HSCOs assist in the delivery of an efficient and effective referral service for vulnerable adults, sometimes with complex needs and / or difficult and unpredictable situations. Applicants should have a good understanding of the variety of services that are available to meet the individual's needs. *
