Pilot Fixated on Intercept, Lost Track of Altitude

An F-15C pilot’s fixation on intercepting an adversary during a training engagement, and subsequent failure to focus on the aircraft’s instruments as he entered cloud layers, caused the fatal June 15 crash over the North Sea near the United Kingdom, according to an Air Force investigation released Nov. 23.
