We are looking for a Short Breaks Worker to join the team on a fixed term contract for 12 months, working 37 hours per week.


The starting salary for the role is ?25,991, subject to satisfactory completion of a probation period (pro rata for part time workers).

Job Purpose

The Short Breaks Service operates citywide and provides both planned and unplanned short breaks for disabled children and young people in order to promote growth, development and inclusion, as well as providing support for families and carers.?


Team members work with a wide range of colleagues both internally and externally, to ensure the Local Authority meets its statutory duties under the Children?s Act, as well as maintaining and forging business relationships with commissioning groups and other relevant stakeholders.


The post holder provides support and guidance to link workers and sitters, and is responsible for reviewing ongoing work with disabled children and young people. They may also review care plans and risk assessments in partnership with a senior member of the team, to ensure that the support provided is person centred, outcome focused and encourages independence.

ESSENTIAL REQUIREMENTS:?

  1. Knowledge of person centred approaches and their application to best practice;
  2. Experience of working with disabled children, young people and their families;
  3. Experience of undertaking specific pieces of work relating to care plans and risk assessments;
  4. Ability to prepare case records, reports and simple correspondence whilst ensuring confidentiality.
