Information About the Consent & Survey
The consent and survey are in English and Spanish, select your language preference at the top of the form. We would like to hear from adults and from youth 13-17 years of age who live in Harlem, the South Bronx, and Washington Heights. Adults need to sign the electronic consent and then proceed to the survey link. For youth 13 to 17, parents or guardians will need to electronically sign a consent for their child and then the child will agree to take survey by signing the “Child Assent” form for themselves and proceed to the survey link. A copy of your signed consent/assent will automatically be sent to your email.
