Fatty Liver Disease Is Now Screened With a Four-Number Blood Score
Since a 2023 renaming, the first check for liver scarring in people with
metabolic risk factors is a calculation from age, two liver enzymes and a
platelet count, followed by a stiffness scan when the score is not low. Connor

Key points

Vomiting blood, black or tarry stools, new confusion or a change in alertness,
abdominal swelling that is new or suddenly worse, or yellowing of the skin or
eyes that is new or worsening quickly calls for emergency care.

In June 2023 international liver societies renamed the condition metabolic
dysfunction-associated steatotic liver disease: fat in the liver plus at least
one of five cardiometabolic risk factors.

The first-line screen, FIB-4, combines age, two liver enzymes and the platelet
count. A score below1.3 makes advanced scarring unlikely; above 2.67 makes it
likely.

Two drugs carry accelerated approval for the inflammatory form with moderate
to advanced scarring: resmetirom sinceMarch 14, 2024, and semaglutide since
August 15, 2025.
Liver fat rarely announces itself. The national digestive disease institute
calls the condition "a silent disease with few or no symptoms," and estimates
that about 24% of American adults have it.

Because symptoms arrive late, the medical system relies on arithmetic. A
routine metabolic panel and blood count already contain the inputs for the
screening score, which is why guidance now asks clinicians to calculate it in
people at risk rather than wait for complaints.

On the tape
Oct 5 A study of 25,736 community-dwelling adults at 18 locations in China,
published in The Lancet Gastroenterology and Hepatology and highlighted by a
sister review journal, found steatotic liver disease in 43.7% using a
stiffness-and-fat scan, and liver stiffness of at least 8 kilopascals in 4.4%.
Why it matters: the figures come from the baseline of a cohort, so they
describe prevalence and point to metabolic dysfunction and alcohol as risk
factors without proving cause in any individual.
Oct 6 A post hoc analysis of a semaglutide obesity trial, presented at the
European diabetes association's 2026 meeting, reported mean liver fat falling
from 8.8% to 3.1% at week 72 in a 55-person imaging subgroup; 23 of 26 with
baseline fat above 5% fell below it. Why it matters: liver outcomes were not
prespecified and no placebo comparison was reported, so the result is
exploratory.
Oct 6 A drug developer announced three poster presentations for the American
Association for the Study of Liver Diseases annual meeting, set for November
5–9 in Denver, and said phase 3 results for its candidate are expected in the
fourth quarter. Why it matters: the field's main annual data event is a month
away; the in-window news is mostly announcements of what will be shown there.
Oct 8 A search of the Federal Register found the newest FDA document
mentioning steatohepatitis is a January 27, 2026 patent-term notice for
resmetirom; no new agency action appeared from September 30 onward. Why it
matters: the two accelerated approvals described below remain the governing
drug labels.
Before anything else
The federal medical encyclopedia advises emergency help for vomiting blood or
rectal bleeding, confusion or a change in alertness, abdominal swelling that is
new or suddenly worse, and jaundice that is new or worsens quickly. It
separately lists black, tarry stools and vomit that looks like coffee grounds
as reasons to call 911. These are signs of advanced disease or bleeding, not of
early fat in the liver, and a screening score has no role once they appear. No
prescribed medication should be started, stopped or changed except with the
prescriber.


Why the Name Changed in 2023
The old terms were nonalcoholic fatty liver disease and nonalcoholic
steatohepatitis. A process led by the American, European, Latin American and
Asian Pacific liver associations replaced them in June 2023 after a multi-round
survey of more than 200 experts; about 85% ultimately approved the
recommendation, according to the American association.

The new umbrella term is steatotic liver disease. Within it, metabolic
dysfunction-associated steatotic liver disease, abbreviated MASLD, requires
liver fat plus at least one of five cardiometabolic criteria. Those are a body
mass index of 25 or more, or 23 in Asian populations, or an elevated waist
measurement; elevated fasting glucose, an A1c of 5.7% or more, or diabetes;
blood pressure of 130/85 or higher or treatment; elevated triglycerides or
treatment; and low HDL cholesterol or treatment.

The renaming changed the definition from an exclusion to an inclusion. The old
name described what the disease was not; the new one names the metabolic
dysfunction that is present. A separate category, MetALD, covers people who
meet the metabolic criteria and drink more than 140 grams of alcohol a week for
women or 210 grams for men. The inflammatory form, formerly NASH, is now MASH.

By the numbers

How the Four-Number Score Is Calculated
FIB-4 is a ratio. A university hepatitis program's calculator states the
formula as age multiplied by the enzyme AST, divided by the platelet count
multiplied by the square root of the enzyme ALT. Age is in years, both enzymes
in units per liter, and platelets in billions per liter.

The score was first validated in hepatitis C, where that calculator still uses
cutoffs of 1.45 and 3.25. Liver-disease guidance for the metabolic form uses
1.3 and 2.67. The American association's fellow network summarizes the 2023
practice guidance: below 1.3 is low risk and can be followed in primary care;
1.3 or higher calls for a second test; above 2.67 makes advanced fibrosis
likely.

Age changes the reading. For people over 65, the same summary uses 2.0 as the
lower threshold, and for people under 35 it calls the score less reliable and
recommends an additional test.

The American Diabetes Association's 2025 consensus report goes further on who
should be checked: "Consider screening all individuals with prediabetes and
type 2 diabetes for MASLD." It recommends repeating the score every one to two
years when it is below 1.3, and states that the condition "affects over 70% of
individuals with type 2 diabetes in the US." That overlap is why a diabetes
body, and not only the liver societies, now publishes screening advice for the
liver.

The FIB-4 score below which advanced liver scarring is considered unlikely,
according to liver and diabetes guidance for the metabolic form of fatty liver
disease. A score at or above it leads to a stiffness scan or a blood fibrosis
panel. The inputs — age, AST, ALT and platelets — come from a routine metabolic
panel and blood count, so the score can be calculated without a new test.


What the Scan Adds and What the Labels Require
The second step is usually vibration-controlled transient elastography, a scan
that measures liver stiffness in kilopascals. In the American association's
summary, below 8 is low risk, 8 to 12 intermediate and above 12 high. The
diabetes association's report treats a reading above 8 as grounds for referral
to a liver specialist. MRI-based tests and biopsy are reserved for uncertain or
high-risk results.

Treatment is defined by the labels. FDA's March 14, 2024 approval of
resmetirom covers adults with the inflammatory form and moderate to advanced
scarring, without cirrhosis, as an add-on to diet and exercise. In its trial of
888 patients, resolution without worsening scarring at 12 months ranged from
24% to 36% on the drugs against 9% to 13% on placebo, depending on the
pathologist reading the biopsies.

Semaglutide received accelerated approval for the same population on August
15, 2025, with a reduced-calorie diet and increased physical activity. At 72
weeks, steatohepatitis resolved without worsening fibrosis in 63% on the drug
against 34% on placebo.

Both approvals rest on biopsy findings rather than outcomes such as cirrhosis
or death; confirmatory trials continue. Whether either drug suits a given
patient is a decision for the prescriber.
