Bloating Has a Short List of Testable Causes

Bloating, fatigue, difficulty concentrating and weight that will not move are the four most common complaints in adult practice, and they are also the four least specific, which is why almost any explanation can be attached to them.

Before anything else, the symptoms that do not wait. Unintended weight loss, blood in the stool, black or tarry stools, difficulty swallowing, persistent vomiting, a palpable lump in the abdomen, or new and persistent digestive symptoms beginning after forty-five all warrant prompt evaluation rather than a dietary experiment. Anemia found on a routine blood test belongs in the same list.

And one standing rule: nothing here is a reason to stop or change a prescribed medicine. Several common prescriptions cause bloating and fatigue directly, and the productive step is a review with the prescriber rather than a decision taken alone.

With those set aside, the condition being described in dramatic terms does exist. Auto-brewery syndrome, in which fermenting organisms in the gut produce enough ethanol to raise blood alcohol, is documented in the medical literature, usually in people with an identifiable predisposition — recent broad-spectrum antibiotics, diabetes, liver disease or an anatomical abnormality of the gut.

It is also extraordinarily rare. The literature consists largely of individual case reports, and diagnosis requires a supervised carbohydrate challenge with serial blood alcohol measurements, because the whole point is to demonstrate alcohol appearing where none was consumed.

A rare condition that produces common symptoms is the ideal vehicle for a claim of this kind, because the symptoms match almost everyone and the rarity is never quantified.

The useful response is not to dismiss the symptoms, which are real and frequently have a findable cause. It is to work through the short list of causes that can actually be tested for — and to do it in the right order, because one of those tests stops working as soon as a food is removed from the diet.

What Bloating Actually Is

The sensation and the measurable distension are two different things, and separating them narrows the possibilities immediately.

Bloating describes a feeling of trapped pressure. Distension describes a visible increase in abdominal girth. Many people have one without the other, and the combination present points toward different mechanisms.

Gas volume is the obvious candidate and is usually not the answer. Studies measuring intestinal gas have generally found similar volumes in people with and without symptoms, which points toward handling and perception rather than production.

Swallowed air contributes more than most people expect. Eating quickly, carbonated drinks, chewing gum and chronic nasal obstruction all increase the volume entering the stomach, and this portion of the problem responds to habit rather than to diet.

Transit is the second mechanism. Contents moving slowly ferment longer and distend more, which is why constipation and bloating travel together so often.

Perception is the third and is well documented. Heightened sensitivity of the gut wall means ordinary volumes register as painful, and this is a defining feature of irritable bowel syndrome rather than a failure of nerve.

Diet composition contributes through volume rather than through any single villain. Fermentable carbohydrates found across a wide range of ordinary foods draw water into the bowel and are fermented in the colon, and the effect is dose-dependent, which is why quantity and combination matter more than the identity of any one item.

Posture and muscle behavior form a fourth, less familiar mechanism: in some people the diaphragm contracts and the abdominal wall relaxes after eating, pushing the contents forward and producing visible distension without any change in volume.

The Test That Stops Working When Food Is Removed

Of everything in this area, one sequence matters more than the rest, and getting it backward is the most common and most costly error.

Celiac disease is an immune reaction to gluten that damages the small intestine, and it produces exactly the symptoms under discussion: bloating, fatigue, weight change and difficulty concentrating, alongside anemia and bone loss.

The serologic test looks for antibodies produced in response to gluten exposure. Remove gluten and the antibodies fall, which is the desired outcome in treatment and a disaster in diagnosis.

Someone already gluten-free who wants testing faces a supervised reintroduction over a period of weeks, which is unpleasant and is the reason clinicians press so hard on doing the test first.

A positive blood test is not the end of the sequence. Confirmation in adults generally requires an endoscopic biopsy of the small intestine while still eating gluten, because the treatment is lifelong and the diagnosis needs to be solid before it is made.

Genetic testing is available and is useful in one direction only. Absence of the associated variants effectively rules the condition out; presence rules nothing in, since most carriers never develop it.

The practical rule is short: blood test before diet change, not after.

Where Fermentation Genuinely Happens

Fermentation in the gut is normal, useful and constant, which is what makes the dramatic version of the story plausible enough to travel.

Bacteria in the colon ferment carbohydrate that the small intestine did not absorb, producing gases and short-chain fatty acids that the body uses. This is the ordinary state of a healthy gut.

Small intestinal bacterial overgrowth describes fermentation occurring higher up, where it produces gas closer to the stomach and earlier after eating. It is diagnosed with breath testing, and both the test and the threshold for calling it positive remain contested among specialists.

Carbohydrate malabsorption is the more common and better-established explanation. Lactose intolerance is measurable by breath test and affects a large share of adults worldwide; fructose and sorbitol produce similar effects at sufficient doses in almost anyone.

Ethanol production is where the rare condition sits. Producing enough to be measurable in blood requires an unusual organism load and an anatomical or metabolic predisposition, and confirming it requires observed testing rather than symptoms.

Antibiotics are the common thread in several of these. A course of broad-spectrum treatment changes which organisms dominate for weeks to months, and both overgrowth and the rare alcohol-producing states are frequently described in the literature as following one.

The distinction that matters is between fermentation as a normal process, fermentation in the wrong place, and fermentation producing a substance it normally does not. The first needs nothing, the second is testable, and the third is rare enough that it is a diagnosis of exclusion.

What Fatigue and Brain Fog Do and Do Not Indicate

The non-digestive symptoms attached to these claims have their own list, and it is mostly not gastrointestinal.

Thyroid function, iron status, vitamin B12 and folate, blood glucose and kidney function are the standard first blood tests when fatigue is the complaint, and each identifies a treatable cause.

Weight change belongs in the same panel rather than in a separate story. Thyroid disease, some medications and sleep disruption all move weight in ways that no dietary explanation covers, and each is identified by a test rather than by an elimination.

Sleep is the single largest contributor and the most under-investigated. Obstructive sleep apnea produces daytime fatigue, difficulty concentrating and weight gain together, and it is common and treatable.

Medication is the third. Antihistamines, some blood pressure drugs, several antidepressants and anything with sedating properties produce exactly this cluster.

Alcohol itself deserves a plain mention, since it is the most common reason for a raised blood alcohol level and for exactly this symptom cluster, and it is the explanation any clinician will consider first whatever the patient reports.

Mood matters and is frequently the elephant in the consulting room. Depression and anxiety produce fatigue, poor concentration and appetite change, and they are more common than any of the gut conditions discussed here.

The point is not that gut health is irrelevant. It is that a symptom cluster this broad is the beginning of an investigation, and the investigation runs through blood tests and a sleep history before it reaches fermentation.

The composite point is that the dramatic version of this story describes a real condition that is extremely rare, that the common causes of the same symptoms are few and testable, and that the order of the steps matters because the most important test is destroyed by the intervention people try first.
