The focus of the work done by Social Care Reablement is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.

The JackDawe team provide a specialist service supporting citizens with a range of complex needs, to enable them to continue with everyday activities and tasks that are meaningful to them. This also encourages independence and can reduce the impact of behavioural and psychological symptoms associated many complex needs. JackDawe service also supports in practical ways, such as medication management, personal care and home management.

Both teams encourage independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen?s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.

Team Information:
The Whole Life Disability Programme supports citizens with physical and learning disabilities from childhood right through to adulthood to ensure a smooth transition between services. The team works closely with Health colleagues to commission and support citizens in a range of services including community support, residential and hospital settings.

Children?s Social Workers work citywide across Nottingham, to provide support to children and young people (and their families) who are identified with a disability. The team carries out assessments and reviews and then implements care and support according to their assessed needs and in line with current legislation. Workers can also deal with Safeguarding concerns and work closely with other practitioners and teams on Child Protection plans where necessary.

The successful candidate will work within current legislation, the City Council?s policies and core values, and Departmental policies and procedures to ensure the delivery of appropriate services to children and their families, according to their assessed needs. This will include assessing the needs of children and their families against the Department?s agreed threshold criteria and in accordance with National Guidance within set timescales, and the creation and implementation of culturally appropriate care plans that demonstrate flexibility and innovative use of resources.

The successful candidate is expected to have completed ASYE or equivalent. They will take part in team duty systems, providing an immediate and responsive service as the need arises in emergency work, and represent the Authority in court routinely, providing reports and acting as the lead witness and providing support to less experienced colleagues and witnesses as required.

More experienced candidates are expected to carry a full and complex caseload, work with minimal supervision and provide guidance to Social Workers level 1-2 and unqualified staff, leading Child Protection investigations.
