SPRINGFIELD – 1099-G tax forms are now available for claimants who received unemployment insurance (UI) benefits in 2025, the Illinois Department of Employment Security (IDES) announced today. Claimants who collected UI benefits last year will need the 1099-G tax form to complete their state and federal tax returns. Also called ‘Form 1099-G, Certain Government Payments,’ this form is necessary for individuals who received state or UI benefits.

Claimants who opted to receive their 1099-G tax form electronically have received an email notification with instructions to access the form from their IDES account.  Claimants who opted not to receive their 1099-G tax form electronically will receive a hard copy paper form via regular mail at the address on file with the Department.  

All claimants may also access their 1099-G tax form online by logging into their IDES account or calling Tele-Serve at (312) 338-4337.  The last seven years of 1099-G tax forms are also available via their online account.  Please note that all IDES online accounts require ILogin verification. If an individual has not received the form through the mail or is unable to access it online or through Tele-Serve, they can schedule an in-person appointment for pickup at one of the IDES local offices.  Appointments for pickup can be made online Monday – Friday between the hours of 8:30am – 9:30am or 4pm – 5pm.  Appointments for 1099-G tax form are for pickup only.  If further assistance is needed, please call the Scheduling Hotline at (217) 558-0401.
