2026 Provider Enrollment Application Fee Set at $750

The Texas Health and Human Services Commission (HHSC) has set the 2026 Texas Medicaid enrollment application fee for institutional providers at $750 (increased from $730).

The 2026 enrollment application fee amount applies to enrollment applications that are submitted on or after Jan. 1, 2026, through Dec. 31, 2026.

*Certain Providers Must Pay the Fee*

Providers must pay the application fee if they are:


* Enrolling in Texas Medicaid or the Children’s Health Insurance Program (CHIP) "for the first time."
* "Reenrolling" in Texas Medicaid or CHIP.
* "Revalidating" their Texas Medicaid or CHIP enrollment.
* "Adding a new practice location."

*Note:* Providers that have already paid the application fee to Medicare or another state’s CHIP or Medicaid program have met the fee requirement and do not have to pay the fee to Texas Medicaid. Proof of payment must be submitted with the application.
