*National Core Indicators In-Person Survey *

The Developmental Disabilities Community Services Division with the University of Washington are conducting the National Core Indicators-Intellectual and Developmental Disabilities In-Person survey. The purpose of this survey is to gather feedback on services.

Potential participants are adults with a developmental/intellectual disability who receive at least one paid service from Developmental Disabilities Community Services.  Participants served in a Residential Habilitation Center or a State Operated Living Alternative may also be identified to participate. 

A surveyor from the University of Washington will call randomly identified adults, and their identified advocate when applicable, to ask if they are interested in being interviewed. The survey is completely voluntary and choosing to participate or decline will have no impact on their services.

All information collected is confidential.  No identifying information will appear in any report.  There is no way for anyone to determine how a participant answered any questions on the survey. 

For those choosing to participate, the interview will be scheduled on a day, time, and location convenient for them, either in-person or via Zoom video conferencing. The interview takes approximately 60-90 minutes.  Participants are welcome to have a family member, friend or advocate of their choice with them during the interview.
