For those working in rural communities, the CMS Rural Health Transformation Initiative ([link removed]) presents opportunities for reshaping care delivery, community partnerships, and access to services. But it has also left many organizations with questions about how it will be implemented and the varying approaches across states and regions.

Led by Health Equity Community Collaborative ([link removed]) Co-Founder & Core Partner Denise Octavia Smith, MBA, CHW, PN ([link removed]) , this month’s community gathering will include a focused discussion about this initiative - how local leaders can get involved, advocate for communities, and influence its direction.

Join us on Thursday, October 30th, 3:00-4:30 pm ET, when Denise will be joined by a member of the National Governors Association ([link removed]) to:
* Explore initiative challenges and opportunities: Discuss how we can advance health equity through the Rural Health Transformation Initiative
* See what’s happening across the country: Hear from community members on how the initiative is unfolding in different regions
* Share tools and resources: Learn what’s available to stay informed about initiative progress and advocacy
* Connect and relationship build: Foster solidarity and support through small group discussions and collective reflection
