Reminder: Please Use the Most Current CAH Forms and Templates

The Court of Administrative Hearings (CAH) has updated all forms and templates to reflect our agency name change (effective August 1, 2025) and standards. The use of outdated forms may cause confusion or delay processing.

To ensure accuracy and consistency, we ask that you always use forms from the CAH Forms and Filing page, instead of using older versions saved to your own system.
