Help Us Identify Gaps in the Behavioral Health System

You are invited to complete a survey designed to help us identify gaps in Wisconsin’s mental health and substance use services system. Your feedback also will help us identify strategies to address the gaps.

We are conducting this analysis in partnership with the University of Wisconsin’s Population Health Institute.

For the survey, you may choose to answer only mental health related questions, only substance use related questions, or both. It should take 30 minutes or less to complete the survey. Participation is anonymous and voluntary.
