Over the weekend, the news broke that Biden has been diagnosed with stage 4 prostate cancer.

First and foremost, our prayers for President Biden and his family.

For those of you who don’t know, this type of diagnosis is very difficult for the patient and their family, especially as they try to understand their diagnosis and treatment options.

I got lots of conflicting advice on my own prostate cancer challenge. I had to go find the best data. And for me, our decision was surgery. As we surgeons say, “the chance to cut is the chance to cure.”

Prostate cancer is common. This year approximately 315,000 men will be diagnosed, with 35,000 prostate cancer deaths. On rare occasions, men do have a low PSA with advanced prostate cancer.

Here ([link removed]) are the current USPSTF recommendations.

However the emphasis is on individualized care appropriate for the patient. It would only be appropriate that the Leader of the Free World--especially if you considered your patient to be vigorous and likely to live for a long time--would be watched more closely.

More data is always better. As there are difficult decisions to be made.

Care in 2025 is much more sophisticated and successful than even just a decade ago. But it is complicated. Most notably, President Trump’s and Obama’s physical reports often included a genito-urinary exam and PSA levels. Biden’s physical reports do not. (Click here to view pdf with physical reports.) In my case, we relied on more reliable genomic testing of my actual tumor to make the best decisions. These usually take 2-6 weeks.

Hopefully President Biden’s case will have genomic testing. Genomic testing can decide the best hormone and chemotherapy options. If prostate cancer is caught before it metastasizes to the bone, surgery to remove the prostate or radiation to destroy can be curative.

Prostate cancer often feeds on the hormone testosterone. Androgen Deprivation Therapy (ADT) cuts the amount of testosterone by chemical or physical castration. There are three basic options 1) orchiectomy (rare in 2025) 2)drugs that block the production of testosterone, 3) drugs that block testosterone from fueling the cancer cell. Patients with metastatic prostate cancer have multiple treatment options to control the disease.

No treatment is without side effects. In elderly patients with dementia, ADT can harm learning, memory processing and executive decision-making.

The lack of true transparency when it comes to the leader of the Free Worlds health is concerning. Our nation and our state need leaders who are less concerned with themselves and more concerned with bettering our state and our nation. It is vital that we we have transparent and competent leadership ready to fight and ready to lead.
