* *Summary:* Effective July 1, 2025, MnCHOICES initial assessments will be valid for up to 365 days from the initial assessment interview date to establish program eligibility.

 

Waiver-funded assistance for community emergencies or disasters [ [link removed] ]

* *Audience:* Lead agencies and service providers
* *Summary:* DHS may approve lead agencies to authorize the use of respite services or residential services during a community emergency or disaster. This waiver-funded assistance applies to people who receive home and community-based services (HCBS).





DSD messages

Grant opportunity to reduce hospital discharge delays for medically complex pediatric patients [ [link removed] ]

* *Audience:* Home care nursing agencies
* *Submission deadline:* 4 p.m. on April 18, 2025
* *Summary:* DHS is accepting applications from licensed home care nursing agencies to develop a pilot program that addresses hospitalization and discharge delays for medically complex pediatric patients.
