Summary: DHS seeks information to better understand the availability, accessibility and quality of ICS services in Minnesota.

Summary: DHS published a list of all HCBS settings actively licensed by the Minnesota Department of Health as an assisted living facility and enrolled with DHS to deliver CL services. This list is a resource for authorizing CL services through the Brain Injury (BI) and Community Access for Disability Inclusion (CADI) waivers.
