“Cars are leaving in 10 minutes,” said the project coordinator on my handheld radio. I cursed under my breath. My patient, Noor*, was crying in pain. She was anemic and bleeding heavily—she was losing blood quickly, and we were expecting the situation to worsen. She had traveled for days amidst active conflict to reach the Doctors Without Borders/Médecins Sans Frontières (MSF) facility in Mosul, Iraq, due to complications with her pregnancy. The fetus had anencephaly—a fatal abnormality that causes death in utero or almost immediately after delivery. Sending her home would have put her life at risk.
I radioed back, “Can we hold the departure time? I need more time.”
Ongoing fighting meant that we had to leave the hospital by late afternoon each day, with our local colleagues remaining overnight. None of us wanted to leave before we could help Noor. We all understood that once we left for the day, the remaining team would lose critical resources. It would be more difficult to maneuver things in the operating room, different skills and expertise would be unavailable, and we wouldn’t have enough hands to help Noor. It would be catastrophic if she delivered overnight. Every second was critical.
