These are a unique tool for individuals responsible for oversight, management and governance of practitioner performance concerns, including Chief Medical Officers, medical managers and HR personnel. Collaboration for safer and sustainable maternity care – resources now available [[link removed]]

On Wednesday 2 October 2024, we held a collaborative regional event for the South West. Member initiatives to improve maternity safety across the region were shared, alongside insights from claims data, Early Notification case themes, and an update on the Maternity Incentive Scheme. Case of note: Karen Spellman v. Portsmouth Hospitals University NHS Trust (High Court, 31 July 2024 – Sweeting J.) [[link removed]]

In this case, which examines allegations of clinical negligence in a trust’s failure to identify Cauda Equina Syndrome (CES), the court considered divisive claims, with experts disagreeing on the MRI interpretation and whether CES was present. Read more about this case. [[link removed]]

Key initiatives involve implementing the Patient Safety Investigation Response Framework, training Safety Champions, and fostering a just, learning culture. World diabetes day 2024
