Summary: DHS requests feedback on barriers and potential solutions to help people move from acute care settings to nonacute care settings.

Audience: Lead agencies (i.e., counties, tribal nations and managed care organizations)

Effective date: Nov. 4, 2024

Summary: The complex transitions referral form will be available to all lead agencies.

Audience: Providers of in-scope service for EVV, regardless of payer, that provide personal care services, including some waiver services, home health services and other interested parties

Effective date: Jan. 1, 2025

Summary: DHS will require providers to follow a specific process for all people enrolled in the Safe at Home program who use EVV.

Audience: CFSS agency-model providers

Effective date: June 1, 2025

Summary: DHS will begin to collect CFSS cost-reporting data. We are beginning CFSS cost reporting because the Minnesota Legislature directed us to develop and implement a cost review for organizations that provide CFSS agency-model services.

Summary: DHS updated policies to create a new workflow for the MnCHOICES Assessment and Support Plan for people enrolled in managed care and on a disability waiver.

Audience: People who receive personal care assistance (PCA), Consumer Support Grant (CSG) and CFSS services, families, providers, lead agencies and other interested parties

Summary: DHS began the transition from PCA and CSG to CFSS on Oct. 1, 2024. We will provide regular updates early in the transition.

Summary: Effective Oct. 18, 2024, the WMS is available for users to resume work. Allocation changes and updated consumer directed community supports (CDCS) budgets are available.

Summary: Join DHS and Minnesota Networking in Education and Assistive Technology (MN-NEAT) for a focused training about integrating assistive technology and remote support into the MnCHOICES assessment process.
