None of us can predict what tomorrow may bring. What if you weren’t able to speak for yourself? Do you have a written plan stating what healthcare treatments you would or would not want?
Creating a good care plan involves more than completing an advance directive—it starts with a conversation. Join us for a free presentation on how to prepare advance directives and make end-of-life healthcare decisions for yourself. Our team will ask questions to clarify what’s important to you about the care you receive and go over any existing advance medical directives with you. Together, we will explore how our values, beliefs and wishes play a role in how we define living well.
Advance directives will honor your medical choices and be a legal guide to help your healthcare providers, family and friends know exactly what you would want when the time comes that you are unable to speak for yourself.

- Healthcare Proxy
- Medical Power of Attorney
- Living Will
- Do-Not-Resuscitate (DNR)
- Medical Order for Life-Sustaining Treatment (MOLST)
